Request for Scheduling Cards:

Scheduling Appointments:
We will assist your patients with scheduling a procedure. Our schedulers are available for calls Monday through Friday from 8:30 a.m. to 5 p.m. To ensure a faster response time, please include your patient's contact information, symptoms, medical information and any other pertinent information.

Please inform your patient that if they do not receive a call from one of our schedulers within 2 business days to give us a call.

To Schedule an Appointment please do one of the following:
1.  Download the GI Patient Referral Form and fax it to 253-272-0811. One of our schedulers will call your patient to set up their appointment. You may also CLICK HERE to request an appointment online.
